That's not too hard to see by yourself, you can just open it in paint or any other image editing program and pick the color. PES color slider only goes up to 63, so I think you have to divide by 4 to get the right color for pes.

Anyway, here it is:

Man. City Home:
Blue: 150, 195, 226 (for PES it would be around 38, 49, 57)

Man. City Away:
Claret: 134, 42, 91 (for PES it would be around 34, 11, 23)

Juventus Away:
Yellow: 240, 189, 38 (for PES it would be around 60, 48, 10)

I have some finishing touches todo with tom ince but ill be posting benteke later tonight

I have PES 2012 in my PC (WIN7) and each time i try to install patchs i downloaded from konami site. This is the message i get:-

No prior installation of Pro Evolution Soccer 2012 was found.
Please re-install it and then retry the installation of Pro Evolution Soccer 2012 patch 1.02.
