Good players with high condition/form stat?

Besides Arshavin, which is an obvious pick, which players have high stats (80+ overall) coupled with a high form stat? I'd like to hear at least a suggestion for each position.

One of the best methods is to get a coach who has top level stats for form, any player with a form rating of 5 or higher should be consistent depending on his performances with this coach.
