How to make substitutions?

I've been playing PES 2012 for a couple of days now and can't figure out how to make substitutions. When in a match, I press start (on the Xbox 360) the game pauses but there's no option on the screen for making subs. I've tried this in several matches and realise I must be doing something wrong. Help!

When you press start, go into "Game Plan" from the pause menu, select the player you want to bring off, and then select the player that you want to bring on as a replacement from the names to the side of the players who are playing.
