Any tips on how to pull the dive off this year?

its simple and you will be able to pull it off everytime you try to execute it :

L3+R3+L1+L2

are they designed the dive to be always unsuccessfull ? (yellow card)

or we have to use it in perfect timing ?

Perfect timing I'd say unlike PES11 where you'd always get a Yellow.
I won a free kick outside the box once while playing on top player and Granero got yellow carded for diving in a online match but it wasn't a perfectly timed I got to admit.
