It happens if you keep playing someone continuously game after game, never resting them. Haven't had it happen myself, but a thread like this was made a while back and a few other people have had the same experience so I don't think it's a fake.

Are you sure the Dominican Republic are not in the nationalities list? I know they are not in the actual playable teams, but if you create a player, when you choose their nationality there's a huge selection of countries to associate them with.
