All I can really tell you is: practice makes perfect.
First of all, the scan needs to be very good. Ie; not smiling, no shadows on the face and an even colour all over. As a rule, if you don't instantly recognise the player in the scan, it won't be any good.
Then use a google image search of the player to get the shape of the head and face right, making sure it still fits the scan.
Then adjust skin colour and calibration. This can be quite difficult as once the face is saved, the game reduces the quality of the face. Basically make sure that when you rotate the player, the skin tone of the face is no different to the rest of the head. This prevents the players from having a glowing or darkened face. I would advise against using the default skin colours as anything other than a guide. Pick one that is close to what you want and adjust it. They usually need to be made darker/duller to be the same as the preset players' skin colour.
Then, when it looks totally wrong, load it up and start again.
