Well for the early sliding he does, the easy way to counter that is to not sprint. Keeping your finger on the Sprint button is a rookie error made by many when in fact you probably only need to use it 30% of the time. But if he's sliding early, keeping the ball close to your player's feet should be able to get the advantage. Playing 1-2's between your strikers should be able to counter his early slides easily too.
Terry and Carvalho at CB work awesome together so your best bet is not to try and dribble through them, but to just pass the ball around and make Chelsea do the running. Once they're out of shape, and the defence is looking out of position, just play into the gaps.
If he's playing 4-3-3 as I assume, your best chance of stopping the solid midfield trio is to be aggressive. Throw in some challenges and try to pressure them, because if you give the likes of Lampard and Ballack time, then they're going to punish you, badly. Always close down possible shots and don't leave any defensive gaps, that's a must against players like Shevchenko.
By making your defence play deep, it may stop any breakthroughs the likes of Joe Cole and Robben are capable of. Maybe set the back line to "C". You could also 'Man Mark' Robben because if you do, chances are, he'll be knackered after the first half and won't be able to offer much else for Chelsea.
I'd probably opt for a 4-4-2 formation, with a team other than Real Madrid I think because defensively they aren't very solid. Possibly Barcelona, Arsenal, Inter. Here is what I would play:

This way the Second Striker acts as a AMF midfielder as well and helps link the play between the midfield and attack. Remember though, you'll need to choose a strong all around team, who have solid defending, as well as a strong attack. Have a look around the teams, and see if they have 2 beasts that can play the DMF-duo, if not then you could improvise and opt for a DMF and play a CMF slightly ahead. Pacey wingers is a must, no two ways about it mate, we all know Ashley Cole likes to push forward from time to time so you can capatalise on this by playing fast wingers. Maybe have a tall target man if necessary to get on the end of the crosses they could swing in.
