I haven't been subbed out with any regularity since my third season. My player started with fantastic stamina and primarily plays a less commonly subbed position (DMF), though.

It should be available on the main menu of BAL the minute you sign a professional contract, unless the PC version is dramatically different from all the others.

Players aren't usually too negotiable on salary unless their affection for the club is REALLY strong. Most of the time, just pay them what they want, but either offer long contracts if they're fairly young, or play them a lot and try to flip them.
