In PES 2016 (on the PS4 at least) it does it automatically. You don't do it manually like in previous games. I think the only place to check would be the notifications window before you get to the main menu.

Yes. I've had 2 players achieve 'Grand Master' status, not sure about Legend or Bandiera. As to how it happens.... I'm pretty sure it's just random. Renato Augusto gained it in my first season with Corinthians, and Felipe Anderson gained it after a couple of seasons with my Bolton team.
